Payment Plans

• Choose between monthly payments or pay-in-full at enrollment

• Pay-in-full option includes a discount

• Monthly payment plans can be paid off early if desired

• Early payoff doesn't qualify for the pay-in-full discount

• Early cancellation available only per our [cancellation policy]

Monthly Payment Schedule

• Payments process automatically on your enrollment date each month

• If you enrolled on the 31st, payments process on the last day of each month

• Need to adjust your payment date? Contact our office

Insurance and HSA Documentation

• Superbills provided for completed visits upon request

• Documentation available for HSA/FSA reimbursement

• Superbills cover visit fees only (not labs or administrative fees)

• Request documentation through our office
